Function / Content

This series consists of a medical certificate book of cause of death from the Tennant Creek Hospital. Certificates are titled 'Form A' and include the particulars of deceased, cause of death, medical and post-mortem details, whether a report was filed to the coroner, and signature of the medical practitioner present at or after time of death. The form was to be completed by a medical practitioner before a body was taken to the mortuary, and a copy forwarded to the Deputy Registrar responsible for the area in which the death occurred. According to Section 34 of the Births, Deaths and Marriages Registration Act, medical practitioners responsible for a person's medical care before death, or who examined the body of a deceased person after death, were required to give notice within 48 hours after the death, in a form approved by the Registrar, of the death to the Registrar, including the particulars required by the Births, Deaths, and Marriage Registration Act Regulations. Medical practitioners were exempted from this responsibility under particular circumstances. This included cases where another doctor had given the required notice, a coroner had been notified of the death under the Coroners Act and was required to give a copy of the notification to the Registrar upon receiving it, or if a child was still-born.
